Diablo V casts you as the heir to Westmarch in a continent overrun by hell

Joe Shely: Dread Commanders with the power of the Dukes of Hell rule greater Westmarch, and the story begins when you enter the Terror Realm and survive.

Diablo V’s player character is the heir to Westmarch, senior game director Joe Shely confirmed at the Diablo V panel at BlizzCon 2026, answering host Johanna Faries’ question of who exactly you are in the game. “As Jennifer said, you’ll be playing the heir to Westmarch,” Shely said. “We’re returning to the Western continent, and Westmarch is the center of Diablo’s invasion. The continent is essentially greater Westmarch. All of it has been overrun. It’s become an outpost of hell.”

Almost nobody is left to help. “There are hardly any humans who haven’t bowed or become cultists,” Shely said. Some powerful humans have been corrupted and raised into Dread Commanders, who “have the power of the Dukes of Hell.” Anyone who resists is put into a Soul Cage or pulled into the Terror Realm, which Shely described as “Diablo’s own corner of hell.”

That is where the plot turns. “The story really begins when you find out that you can enter the Terror Realm and survive,” he said. “While you’re there, you learn about a few other powers that we’re not quite ready to tell you about yet.” As he spoke, the screen behind the panel showed artwork of a hulking winged figure seated on a throne in a red hall, bodies heaped at its feet.

What lets the heir survive the Terror Realm, and how that ties to the bond with Diablo described earlier in the panel, was not said.
